Positive Brain Health

Nature Minds is a Positive Brain Health programme for people living with cognitive change difficulties who want to keep doing the things that help life feel active, interesting and connected.

A change in memory or thinking does not mean giving up on learning, contributing, enjoying new experiences or being part of a group. Nature Minds is designed around what people can do, with opportunities to stay involved, build confidence and enjoy a genuine sense of purpose and achievement.

The Nature Minds Programme Suitability Checklist

The Nature Minds Suitability Assessment Checklist is a short 20 -question screening tool. It is in 2 sections.
The first section tells us how to contact you and that you agree with the limitations of the questionnaire.
The second section is designed to indicate how well a person’s interests, abilities, independence and level of engagement align with being suitable for the Nature Minds programme.

The final result will be emailed to you (as long as you have provided an email address) when you click submit and is a guide to the programme suitability for you. It also sends a copy to Nature Minds.

Your answers are scored from 1 to 5 across the 14 suitability questions, giving a total score out of 70.
A higher score may indicate a strong match for the Nature Minds programme. There are some criteria  that require key participation criteria. If your score falls below the required minimum, your result will recommend a conversation with a member of the Nature Minds team.

If your overall suitability score is low, we also encourage you to contact us, as another Home4All programme may be a better fit or we may well be able to help you explore other options.

Next Steps

Once the assessment is completed, the next step is usually a conversation with the Nature Minds team to discuss your result, clarify any areas that may need further consideration, and answer questions about the programme.

Where the assessment indicates a good fit, this can lead to confirming a place in an upcoming Nature Minds programme.

If further review is recommended, the team may suggest a more detailed discussion, seek additional information, or explore another Home4All programme that may better suit your needs.
